Superbill

A superbill is an itemized document prepared by a healthcare provider that contains information an insurer may need to process an out-of-network claim. For therapy, it commonly includes provider and patient details, dates of service, service codes, charges, and other information required by the payer. The exact fields depend on the claim and insurer requirements.

A superbill is not the same as a payment receipt, an insurance claim, or a guarantee of reimbursement. The client may still need to submit a claim form or provide additional information. The insurer then decides whether the service is eligible and how the plan’s deductible, allowed amount, and coinsurance apply. See Deputy Care’s superbill reimbursement guide for the process and common considerations.
