An Explanation of Benefits, or EOB, is a statement from an insurance company explaining how it processed a claim. It may show the amount submitted, the allowed amount, the amount paid by the insurer, what was applied to the deductible, the member’s responsibility, and any adjustment or denial information.
An EOB is generally not a bill. It is a record that helps the member understand the insurer’s decision. If the amounts seem unexpected, compare the EOB with the superbill, payment records, and claim details, then contact the insurer for clarification. Keep the EOB because it can help you track reimbursement and identify whether more information or follow-up is needed.
